Additional details:

• Funds will be distributed to teachers/interns/staff in the form of a cash gift.

• Teachers/interns/staff do not see individual gift amounts.

• You may use this form as many times as you'd like.

• We will compile and distribute any notes to teachers/interns/staff you would like to include.

• If you would like to leave a note but not give a gift, enter a dollar amount to enable the notes box to save and use the code NOTESONLY in the Scholarship Code section at checkout. This code will apply to your entire cart. Please read the additional instructions in the form.

• If you see a $0.00 that is not changeable, that staff member has elected not to receive monetary gifts. Feel free to leave a note! 

• Please follow the checkout instructions carefully to make sure your gift is processed!

You must be logged in to Normandale's Membership Toolkit portal to access the form.
Forms are found under the My Account menu. Normandale families already have an entry in the directory even if you have never logged in. If you are unsure what email address is associated with your Normandale Membership Toolkit account, or if you have any questions about the form, please email Sheri Rosen.

The gift form is also available through the Membership Toolkit mobile app. You can find it in the top left menu under My Account > Forms.
